As part of the New York Social Care Network, NYREACH now offers medically-tailored meals through our partnership with Epicured. How It Works
Social Care Network members can access this service through their care navigator. Meals are delivered weekly and include: - Breakfast, lunch, and dinner options - Meals designed for specific dietary needs - Fresh, high-quality ingredients - Easy-to-heat preparation
Eligibility
This program is available to Medicaid members enrolled in the Social Care Network who have been identified as having nutrition-related health needs.
